Our Plans:
Individual Plan
- First year – one comprehensive exam & one periodontal exam
- Subsequent year – one annual exam & one periodontal exam
- Two cleanings (in absence of infection)
- One oral cancer screening
- One fluoride treatment
- 4× bitewing x-rays
- One panoramic x-ray
- Individual x-rays (as needed)
- 50% off additional cleanings
- 15% off fillings, core build-ups, oral surgery, root canals, crowns, bridges, partials, and dentures
- 10% off veneers
- $500 off clear aligners
Periodontal Plan
- All services and offerings included in the individual plan
- Three cleanings (periodontal maintenance)
- 20% off scaling and root planing
- 50% off Arestin placement
Family Plan: (+$299 per additional family member annually)
Exclusions and Limitations of the Family Plan:
This program is a discount plan, NOT a dental insurance plan.
It cannot be used:
- In conjunction with another dental plan, dental insurance plan, or financing program such as Care Credit unless it is discussed with the doctor beforehand.
- For treatment which, in the sole opinion of our doctors, lies outside of the realm of their capability
- For referrals to specialists
- For hospitalizations or hospital charges of any kind
- For costs of dental care covered under automobile or medical insurance
- For services of injuries covered under worker’s compensation
→ Membership is valid for one year from date of enrollment.
→ Membership fees are non-refundable and must be paid in full at time of enrollment.
→ Plan is only honored at Chesterfield Dental Studio. It cannot be used at any other dental office
